Chronic disease is a condition that has been present for at least 3 months and is likely to persist needing ongoing care and management. Some of these conditions maybe reversable, such as those related diet, nutrition and physical ailments. The clinicians at Grow Medical Group provide consultations that include a comprehensive medical and social history, collation of past medical and prescription records, a psychical examination, a written management plan, referrals to a multi-disciplinary team that will assist with disease management and set dates for review and adjustment of health goals. Registered patients have access to 4-5 Bulk-billed appointments per year for referrals, scripts and review.
